Please set up my massage table rental at the seminar location for an additional $20. MFR Seminar participants check this box. This includes delivery to/from the seminar locations and set up of the massage table prior to registration, if allowed by seminar staff. NOTE: Most MFR seminars allow set up of the table prior to registration. Please deliver/pick up my rental at my hotel for an additional $20 fee + mileage ($1 per mile to and from our facility for both delivery and pick-up: total for both directions). (Please specify hotel or location below). A credit card and refundable $200 cash security deposit are due upon delivery for renters other than MFR students/clients. NOTE: PLEASE CALL DIRECT TO SCHEDULE TIMES AT 928-496-0091. I will pick up and return my massage table rental to the storage facility. A $200 refundable cash security deposit and valid credit card are due at time of pick up. No charges or holds will be placed on the credit card unless the table is not returned. I am requesting a delivery quote for outside the immediate Sedona area. Please put details of location in last section below.
