RENTAL AGREEMENT/Contract

For purposes of this contract, the owner of the massage tables and/or the agent renting out the massage tables will be referred to as the "Rental Agent", while the person who is renting the massage tables for use will be referred to as the "Renter".

Massage tables rented appear, by the Rental Agent, to be in safe and working order at time of rental, barring vinyl wear/tear which may include vinyl tears or shredding.

It is the Renter’s responsibility to inspect all tables at time of delivery for any damage that may appear unsafe for use. The owner of the massage tables and/or Rental Agent take no responsibility or liability for anything that may occur in the use of the massage table by the Renter.  

Liability of injury or damage of any and all kind is solely the responsibility of the Renter. By signing this form, you, as Renter, agree to these terms.

Included in rental is one massage table and one fitted and/or flat sheet(if requested): Head rest and carrying case will not be left with table unless specifically requested by Renter. It is expected that the massage table be returned in the same working condition as when received by the Renter. 

If damage occurs, the Renter must inform Rental Agent IMMEDIATELY by calling Caden at (928)300-6996, Danny at (928)228-5331, or Becca at (586)873-2543. The massage tables are inspected prior to every rental. However, wear and tear does occur. If, at any time, the Renter feels unsafe (i.e., the Renter hears a cracking sound, or sees damage to the frame/base), the Renter is to stop using the massage table, get off the massage table immediately,  and call: Caden at (928)300-6996, Danny (928)228-5331, or Becca at (586)873-2543. The “damaged” massage table will be replaced with another massage table as soon as possible. No charges will be accrued by the Renter. Safety is the number one concern.

It is expected that the massage table be used by the Renter in an appropriate and safe manner as defined by the massage table manufacturer. Deviation from the massage table’s intended use, as described by manufacturer, places full responsibility and liability on the Renter.

In signing this rental agreement, the Renter acknowledges that:
1. The massage table will remain in the state of Arizona.  
​2. The Renter will contact the Rental Agent to make arrangements and come to an agreement if the table is to be taken out of the Sedona/Camp Verde area.  Rental Agent has right to decline this request.
3. The table is allowed to be removed from seminar location for use where the Renter is staying or can be left in the seminar room at night, as the room is locked during off hours.


RETURN OF TABLE:
It is the responsibility of the Renter to have the folded table with linen placed in the back of the room by the doors to the parking garage for pick-up by the Rental Agent on the last day of the seminar.  If this to be different, it is the Renter’s responsibility to contact the Rental Agent to arrange the appropriate time and location of pick up.


PROBLEMS: If there are any questions/concerns, please contact the Rental Agent, Caden at (928)300-6996, Danny at (928)228-5331, or Becca at (586)873-2543. 
Unless contacted within first 24 hours of rental, it is understood that Renter has possession of the massage table and that it is in proper working order. It is expected that Renter will fulfill the rental agreement as written unless Renter notifies Rental Agent within the first 24 hours of rental. No refund will be given after first 24 hours of rental unless Renter has contacted Rental Agent to discuss special considerations.

All attempts will be made by Rental Agent to ensure a safe and positive rental, however, lack of communication concerning rental issues places responsibility, financial or otherwise, solely on the Renter.


When applicable: By submitting this agreement, you are giving the Rental Agent authorization to charge the below credit card and/or pursue any other means to obtain the replacement cost of any massage table not returned to Massage Table Rentals of Sedona (average cost to replace a massage table is $500-$900+ per massage table - statement provided).
